B4/AFTER SCHOOL PROGRAM

Please fill this form out and attach the B4/After school form from the website in the "B4/AFTER PROGRAM" tab. If you feel uncomfortable adding a credit card on the form, please call or come in person!

There is a one time $99 registration fee but you get a uniform and t-shirt!

Please attach a portrait picture of the student or we can take their picture when you come in.
